How and where do I upload documents?

Uploading documents to your AMO dashboard (before reserving an application)

1. Login to your AMO account.

2. Go to your "AMO Dashboard" page

3. Click on the "Documents" tab in the right corner

4. Upload your file in English as a JPEG or PDF.

 

Uploading documents to your AMO application (after reserving an application)

1. Login to your AMO account.

2. Go to your "AMO Dashboard" page

3. Make sure you sign your clinical agreement (grants you additional access to your applications)

4. Click into your application

5. Scroll down to your documents section

6. Upload your file in English as a JPEG or PDF.
